The thing you need to know is that there are lots of different math courses in a CS degree. Some, you may not get at all; . But there are other math courses that you may take which you will find interesting like Linear Algebra, Discrete math, Probability and statistics, Numerical analysis; Just because you’re not very good at high school math and physics doesn't mean you’re not going to be good at all these courses.
The only thing you need to do is to give a little extra time to these subjects but your success in the field of CS.....

The only thing you need to do is to give a little extra time to these subjects but your success in the field of CS does not depend on your mastery over Math and Physics but it depends on how will you overcome this if you are really passionate about CS.
And that's not even the point. Computer science is a VAST field. There are so many things you can do without math or with only a basic understanding of math. From Computer networking to Computer architecture to web/app development, operating systems. There is a lot to choose from. Unless you plan on getting into research, don't expect to be sitting at a desk solving differential equations all day.
You may work as a web developer, create programming courses, work at a robotics lab, work at a Computer vision and AI lab (there was math here, but it wasn’t very hard) and do some freelance projects here and there.
So if the only thing that's stopping you from getting a CS degree is math and Physics then don’t even sweat it. I'm sure you’ll get through it. Hope this helps!
